Bindery Form (Staff Binding Slip):

  • Fill out either a periodical or monograph Staff Binding Slip depending on your binding needs.
  • On the Staff Binding Slip type the information in the text boxes provided.  Although the bindery will take a hand-written binding slip, typing will cut down on errors and miscommunication.
  • Please fill out a binding slip for each item sent to the bindery.  Fold and insert the binding slip into the item/bundle.

Bind Options:

  • Standard Periodical: (estimated price $6.55 ea.):  Bundled periodicals should not exceed 2.5 inches thickness (measured at the spine).   Collate the bundles from earliest to latest issue with the earliest issue on the top.  Group the items together and tie with string in a t-shape.  NOTE:  Rubber bands can damage the unbound issues.  Make sure to fill out a Periodical Staff Binding Slip for each bundle/item bound.  Bundle:  Single issues of a periodical that are grouped together to form a single volume.  A good way to remember this:  when the bundle returns from the bindery, it will be a single bound volume.
  • Custom Monograph: (estimated price $5.45 ea.):  Monographs should not exceed 2.5 inches in thickness (measured at the spine).Make sure to fill out a Monograph Staff Binding Slip for each monograph bound.

Payment:  Staff members who wish to bind their personal items through Houchen Bindery must pay by credit card.  Please fill out the Request to Pay by Credit Card form and submit the form with your order.

Pickup/Delivery:  Items that are sent for staff binding should be dropped off and picked up at 52 Ellis Library (Physical Processing Unit).  When your items have returned from the Houchen Bindery, you will be notified by Physical Processing.

Note:  Physical Processing will not pick up or deliver items.  The individual staff members are responsible for all transportation, binding information, and bill payment.
