Healthy for Life is an incentive-based wellness program designed to help you earn your incentive and actively manage your health. By completing a personal health assessment, health screening, and 240 minutes of cardiovascular physical activity, primary subscribers to the UM health plans will receive $100 in a tax-favored account to use for medical expenses in 2013 Benefit Plan year. You can receive your health screening through Healthy for Life or by going to your health care provider. If you\u2019ve gone to your provider, you can submit your results by using our <a href=\"https:\/\/umsystem-wellness-resources.umsystem.edu\/healthscreen\/\" >doctor screening form<\/a>.<\/p>\n<p><strong>Earn $100 in 3 Easy Steps:<\/strong><\/p>\n<p>1. Complete your <strong>personal health assessment<\/strong>&#8211;an online questionnaire that helps you understand your health risks and provides you with resources to improve your well-being. (between Oct. 15, 2012 and Apr. 30, 2013)<br \/>\n2. Complete your <strong>health screening<\/strong>, which includes cholesterol, blood sugar, blood pressure, height and weight. You can schedule a FREE on campus screening or visit your health care provider (valid screenings are between June 1, 2012 and Apr. 30, 2013).<br \/>\n3. Track <strong>240 minutes of everyday physical activity <\/strong>in any calendar month (between Oct. 15, 2012 and Apr. 30, 2013).<br \/>\n<strong>FitBit Winners!<\/strong><br \/>\nDon\u2019t miss your chance to win one of our great prizes each week. Enter your email address and <strong>join the program<\/strong>.<\/p>\n<p>3. Use the helpful quick-start guide to help you step by step.<\/p>\n<p>4. If you need help, call Cerner at 877-621-8014.<\/p>\n<p>Already Visit Your Doctor?<\/p>\n<p>If you have had your annual physical, you can complete the <strong><a href=\"https:\/\/umsystem-wellness-resources.umsystem.edu\/healthscreen\/\">Doctor Screening Form<\/a><\/strong> and fax it in! It must be <strong>completed in its entirety<\/strong> to get credit.<\/p>\n<p>Get your screening at home<strong><em> <\/em><\/strong><\/p>\n<p><strong><em>Order a Home Access kit &#8211;<\/em><\/strong> If neither of those options works for you, you may request a Home Access kit by calling 866-352-4372 between 6 a.m. and 7 p.m. CST Monday through Friday, excluding holidays. The Home Access kit allows you to collect a small blood sample from your finger and mail it to the Home Access Health Corporation certified laboratory for processing.\u00a0 It takes 2-3 business days to receive your kit once it\u2019s been ordered.\u00a0 <strong>Kits must be sent back and postmarked by April 15, 2013 to receive credit.<\/strong> You will need to gather your blood pressure, height and weight on your own, as these are required for the incentive.<\/p>\n<p><span style=\"text-decoration: underline\"><br \/>\n<strong>Wellness Informational\u00a0Tables\u00a0Available on Campus<\/strong><\/span><br \/>\nStarting next week, Healthy for Life will have tables set up on campus to provide wellness information. You can learn more about tobacco cessation resources as Mizzou will be smoke-free starting July 1, 2013.
