By completing a personal health assessment, health screening, and 240 minutes of cardiovascular physical activity,\u00a0primary subscribers to the UM health plans will receive $100 in a tax-favored account to use for medical expenses in 2013 Benefit Plan year. You can receive your health screening through Healthy for Life or by going to your health care provider. If you\u2019ve gone to your provider, you can submit your results by using our <a href=\"https:\/\/umsystem-wellness-resources.umsystem.edu\/healthscreen\/\">doctor screening form<\/a>.<\/p>\n<p><strong>Earn $100 in 3 Easy Steps:<\/strong><\/p>\n<ol>\n<li>Complete your <strong>personal health assessment<\/strong>&#8211;an online questionnaire that helps you understand your health risks and provides you with resources to improve your well-being. (between Oct. 15, 2012 and Apr. 30, 2013)<\/li>\n<li>Complete your <strong>health screening<\/strong>, which includes cholesterol, blood sugar, blood pressure, height and weight.
