Mark your calendars for campus flu clinics listed below<strong> <\/strong>. Faculty, Staff and their dependents who are 12 years and older <strong>AND<\/strong> are enrolled in a UM Health Care plan can receive a <strong>FREE<\/strong> flu shot. To do so, please bring your insurance card (for each individual receiving the vaccine) and a <strong>completed<\/strong> <a href=\"https:\/\/uminfopoint.umsystem.edu\/media\/curator\/Wellness\/flu%20info\/2012-13%20Flu%20Shot%20Form.pdf\">Flu Consent Form<\/a>. No appointment needed, just show up!<\/p>\n<p>Not a UM Health Care plan enrollee? That\u2019s ok, those 12 years and older can still receive the vaccination.
